• Duly filled-up application form;
  
  
• Certificate of  Live Birth (NSO certified);
  
  
• Marriage contract for married female applicant (NSO Certified)
  
  
• College Diploma and Special Order Number;
  
  
• Transcript of Records and Special Order Number;
  
  
• National Bureau Investigation (NBI) clearance; and
  
  
• Other documents that the Board may require
